Node.js 里 node:stream 与 web streams 靠 toWeb/fromWeb 互转，但容易踩坑：toWeb 会暂停原 readable、默认策略导致双缓冲甚至内存暴涨、背压取消时抛未捕获的 ERR_INVALID_STATE。附三个 Node issue 的根因、复现与规避，以及该改用 ReadableStream.from 的场景。
